PSORIASIS

PSORIASIS
Psoriasis is red scaly (silvery white) patches which can appear any where on the skin.

CAUSES:
• Unknown.
• Possibly genetically determined.
• Common between 15-30 years.
• Precipitating factors are:
– Psychological emotional stress.
– Cold weather.
– Injury.
– Septic focus.
– Drugs like Steroids, Brufen, Lithium, Anti-malarials and Beta - adrenergic blockers.

SIGN AND SYMPTOMS:
• Usually affects both sides of body symmetrically.
• Well circumscribed red, dry plaques (flat areas on the skin) of various sizes, covered with scales.
• Removal of scales gives rise to pin-point bleeding points.
• Sites can be anywhere on the body especially on e.g. elbow, knees, sacrum and occiput.
• Lesions are coin shaped or circular with central clearing.
• Joints can get affected especially finger joints.
• Nails frequently become pitted and thick with brownish black discoloration.

DIETARY MANAGEMENT:
Avoid:
• Avoid precipitating or trigger factors.
• Avoid food you are allergic to.
• Avoid smoking and alcohol.
• Avoid foods high in fat or sugar.
• Avoid non-vegetarian food – meat, poultry.
DO:
• Warmth may help to control relapses.
• Do regular exercise helps by improving blood circulation.
• Practice yoga will help in reducing stress.

CONSUME:
• Drink plenty of water.
• Eat fresh and raw fruits and vegetables as they provide antioxidants and flavanoids which help in reducing inflammation and boosting up immunity.
• Consume figs and fish oil might help you feel better.
• Eat a well-balanced diet - Proper nutrition will improve general health and reduce the severity of the disease. Do not over eat even healthy foods.
• Limit intake of red meat and dairy products.
• Eat foods that contain psoralen before you walk out in sun - Celery, carrots, citrus, figs, fennel, and parsnips. It makes the skin more sun-sensitive to the positive effects of UV light. But do not over expose to sun.
• Consume oily fish, may help reduce the inflammation associated with psoriasis - mackerel, salmon, black cod, albacore tuna, herring and sardines.
• Have a low-calorie diet if you are over weight - about 1,200 calories per day. Some researches have shown relations between psoriasis and obesity.
• Consume gluten free diet, it benefits individuals with psoriasis (Gluten is a mixture of proteins found in some cereals, particularly wheat) – avoid wheat, rye, barley and oats they contain gluten.
• Increase intake of vitamin D.
• Increase intake of omega 3 fatty acids and reduce intake of omega 6 fatty acids (ratio of 2:1), omega 3 fatty acid has anti-inflammatory properties.
• Food rich in omega 3:
– Canola oil, flaxseed, flax seed oil, walnuts and green leafy vegetables like purslane, fish - Atlantic salmon, Atlantic halibut, Atlantic mackerel, Atlantic and Pacific herring, sardines, bluefish, tuna and cold water fish.
• Reduce (do not completely avoid) intake of food rich in omega 6:
– Cereals, vegetable oils, whole-grain breads, baked goods and margarines, eggs and poultry.
• Consume diet rich in vitamin A and zinc, helps in regeneration of the skin.
• Increase intake of vitamin C, it helps maintain healthy skin.
• Have coriander leaves frequently, its high in vitamin C and helps to maintain healthy skin.
• You can have 3-4 strands of saffron in milk, it is beneficial for skin.
• Basil leaves (tulsi) are said to be helpful in skin ailments, eat 3-4 fresh leaves daily.

CONSULT PHYSICIAN:
• You can take vitamin and cod liver oil supplements under guidance of your physician.
• In case of prolonged duration or change in rash pattern take physicians opinion immediately.
• If symptoms get worse or if you see any Signs of infection (fever, redness of the affected area, pain) occur, see your physician immediately.

Principles of prescribing in Dermatology

(DO’S AND DONT’S)

1.      Detailed history of allergy due to drugs taken in the past     should be taken before prescribing any drug. Also history              of ingestion of other drugs must be obtained to avoid drug   interaction.

2.      Nature of treatment depends on the stage of the disease. More acute the condition less strong the local applications

e.g. lotions for acute conditions, creams for subacute conditions , creams or ointment for chronic conditions.

3.      Vehicle in which the medication is incorporated requires careful consideration as does the manner of application.

          Use of incorrect form of medicament will aggravate rather

          Than improve the condition by causing sensitization.

4.      Areas near the eyes and genitals should be treated with mild remedies because of increased percutaneous absorption.

5.      Adhesive plaster should be used with caution and not close to an acute skin lesion.

6.      Patent or proprietary preparations should be used only after studying their composition, side effects and contraindications.

7.      Ultraviolet therapy should be used with discretion in local treatment as wrong use of it may aggravate the condition.

          It should be avoided in cases with a history of photosensitivity and in lupus erythematosus or antifungal treatment.

8.      H/O previous application of corticosteroids can change the clinical picture of the disease.

9.      X-ray therapy in skin diseases should be left to the expert.

10.    If malignancy is suspected biopsy should be done before starting any treatment.

11.    Associated diseases like diabetes, hypertension, pepticulcer or pulmonary tuberculosis should be excluded before prescribing certain drugs such as corticosteroids.

12.    Avoid all drugs in the first trimester of pregnancy. Use minimum unavoidable drugs in later pregnancy. Strong steroid ointment should be avoided since if applied for a long time. They are absorbed into the blood. Ask for epilepsy as drugs given for the same may interact with some antifungal drugs.

13. Additional drugs may have to be incorporated in the prescription to counteract the side-effects of main drug prescribed antacids when corticosteroids are used.

14. Patient who has been fully treated should be re-examined at frequent intervals to observe renote untoward effects of drug prescribed and to check a release.

15.In contagious skin disease, not only the patient but all the family members and contacts should be examined and if found infective, should be treated simultaneously for scabies.

16. Treatment should be divided into two parts:


Local.    
Systemic.


 

Topical therapy is the mainstay in dermatological practice. Understanding the principles of the therapy is important.

 

17. Many topical agents penetrate the skin barrier and enter the blood circulation. Absorption is more in infants and children and this must be kept in mind with corticosteroids.

18. Hydration of skin before topical application enhances absorption. If local application is not applied in correct strength it may give rise to side effects like irritation, burning.

EXERCISE FOR CERVICAL SPONDYLOSIS

To start with, exercise should be done sitting and the feet must rest on the floor or stool.

Exercises should be done at least 4 times a day.

Mobilization Exercise: To be done only once a day and once only.

1. Head bending forwards.

2. Head bending backwards.

3. Head bending to the left side trying to touch left shoulder.

4. Head bending to the right side trying to touch right shoulder.

5. Head turning to look towards left side.

6. Head turning to look towards right side.

Progression of the exercises: (5 times , increase to 7-10 times).

With ½ kg.weight in each arm :

1. Arms lift forwards and up and down.

2. Arms lift sideways and up and down.

3. Arms lift forwards, part and together.

Instructions:

1. Do not look down to read (ordo any other work).

     Try and bring reading material to the eye level.

2.  All the movements can be performed with practice, by using   

trunk movements.

3.  Use a thin pillow supporting the head and neck, pillow line

Upto the shoulders only.

4.  While lying on sides, head should be in neutral position . Use                                           

One pillow and your hand, to adjust the head, or in addition,

One small pillow to adjust the head.

Do's and Dont's
Try to avoid or overcome the precipitating or trigger factors.
A warm climate may help to control relapses. Coal-tar, steroidal preparation are effective for a short time and should not be continued for long.
Artificial ultraviolet light may help.
There is some evidence to link alcohol with psoriasis. So avoid alcohol altogether.

Raise your general immunity with a good nutritious diet and regular exercise.
